Part A: Participant Information Sheet 

About the evaluation

You recently had a combined health check. We want to hear about your experience of the combined health check, so that we can find out what is working well and identify potential areas for improvement.

We are the Strategy Unit. We are part of the NHS, and provide independent evaluation to help the NHS learn and improve the things that it does. 

What am I being asked to do?

We are inviting you to take part in a short conversation to talk about your experience of the combined health check. During the conversation, we will ask you about:  

  • What you thought about the combined health check
  • How the health check compared to other health checks you may have had in the past 
  • How the health check could be improved.  

If you are happy to take part, a member of the Strategy Unit evaluation team will contact you using the details you provided in the feedback survey, to arrange a time for us to speak. Or, you can attend an online drop-in session at one of the following times: 

17th February 2.00pm - 3.30pm 

19th February 10.30am - 12pm

The conversation will take place over the telephone or by video call (MS Teams). The conversation will last between 20 and 30 minutes.  You can choose to have another person attend the conversation with you, such as a friend, family member or support worker.
 

What will we do with the information you provide? 

We will use what you tell us to write a report about the service. We might include quotes from the conversation in the report. Your name will not be used in the report, and no information that identifies you will be included. The report will be shared with NHS England and the GP practices who are offering the combined health check, but they won’t know who told us what. 

Everything you say in the conversation will be kept private unless the evaluator is concerned about your or someone else’s safety. If this happens, the evaluator might have to tell your GP practice what you told us. 

We will audio-record the conversation to help us write up what you have told us. The evaluator will first check you are happy to be recorded. If you’re not, then we will take notes. Recordings will only be accessed by the evaluation team and the transcription service who write up the recording – and they won’t know who you are.

All information will be handled and stored securely according to  General Data Protection Regulations. The information you give us will be kept for 12 months and then it will be deleted. 

NHS England are the data controller for the personal data processed as part of this evaluation (the Strategy Unit are acting on behalf of NHS England). Do I have to take part?  

You do not have to take part, it is your choice whether you would like to speak to us.  If you do not want to, it will not affect the care provided by your GP practice or any other care, support, or treatment.  

If you decide to speak to us, you don’t have to answer every question, or can take a break. You can change your mind about taking part at any time without giving a reason why. However, if you decide to change your mind after you have already given your information, it might not be possible to remove the things you said from our report. This is because the evaluators will have already started to look at the results.   

We won’t be able to speak to everyone, but if we speak to you, we will provide you with a £20 Love2shop gift card to thank you for your time.

Part B: Participant Consent 

Consent is necessary for us to ensure that you understand the purpose of your involvement and that you agree to the conditions of your participation. If you are happy to proceed our researcher will go through the following consent information before starting the conversation.

  • Have you received and read this participant information sheet, or had it verbally recounted to you? The data from the conversation will be handled as described in this participant information sheet (above).
  • Have you had the opportunity to ask questions?
  • Do you agree to the conversation being recorded for analysis and reporting? In using your quotes from the conversation, we will maintain anonymity and we will not include any identifiable details. If you do not agree to the conversation being recorded, the evaluator will ask if you agree to detailed notes being taken. If you do not consent to any record being made of the conversation, the conversation will not take place.
  • What if you change your mind about participating? You are free to withdraw from participating at any time without giving any reason. However, due to the nature of the process it will not be possible for people to withdraw statements already provided once the information has been collected and analysed.
